ID Badges

Prior to the start your rotation, email the Public Safety Department with your legal name, specialty and your headshot in .JPG format, ch-publicsafety@lcmchealth.org.  

Badge Pick Up Hours: Mon., Wed., Fri., located in the Sidekick Call Center on the 1st floor (ring doorbell)

Badge ID Photo MUST meet all specifications naming requirements listed below. 
LEGAL NAMES MUST BE USED ON ALL ID BADGES.  NO EXCEPTIONS.

  • Format: JPG
  • 150 DPI (minimum 72 DPI, any DPI lower than 72 may not be used)
  • At least 720 (w) x 900 (h) or a 3:2 aspect ratio.
  • Photo should include full head, shoulders and chest.
  • Background should be solid (similar to the background requirements for a passport photo).
  • Background should not be too dark or completely white.
  • Please do not crop out space at the top of your head above hair or along the sides of your arms.
