Reporting Incidents (BSAFE)
At LCMC Health, the BSAFE reporting tool is available to everyone on the healthcare team: employees, providers, and learners. BSAFE helps LCMC Health ensure a fair and just learning culture that focuses on health care qualitypatient safety, and life-long learning

You can access BSAFE at every LCMC Health hospital. The icon is displayed on your PC or EPIC desktop. Login is the same as your EPIC login and password, or you can report anonymously by choosing that option.

All BSAFE reports are reviewed by LCMC Health administrative and healthcare quality leadership. In the vast majority of cases, you'll receive feedback on your report, from Academic Affairs, but keep in mind that reports filed anonymously cannot be responded to. Quarterly, Academic Affairs prepares and distributes the Close to Zero report that summarizes the types and sites of reports, entered by learners, across the LCMC Health organization, helping to identify any opportunities for improvement.

While any type of incident can be reported, the more frequent types include Treatment/ Issue/ Injury for healthcare quality or patient safety opportunities, Professional Conduct for interpersonal or communication opportunities, Medication/ADR/Fluid for pharmacy or fluid-related opportunities, and Safety for public safety or personal injury events like needlesticks.

The BSAFE icon can be found on the computer or laptop desktop of any LCMCHealth managed device:
*Note: for urgent situations or incidents, Never Hesitate and Escalate to the on-call House Supervisor (reachable through the operator) or your immediate supervisor for support and resolution.
