INSTITUTIONAL DEA NUMBERS
Temporary DEA #

  • Each LCMC facility has a unique facility DEA number.
  • The temporary DEA number consists of the LCMC Health facility DEA number (the prefix), followed by the individually assigned 5-digit resident suffix
  • The unique facility temporary DEA number is to only be used for prescribing controlled substances for patients at that specific facility.
  • For Example: You CANNOT use the UMC facility DEA number and suffix when prescribing for patients at Touro or any other hospital outside of UMC.
  • Use of the DEA # for patients other than for specific patients of the facility is strictly prohibited
  • Violations will be reported to both CMO and Drug Enforcement Administration for appropriate disciplinary and/or legal action
