FLU VACCINES HAVE ARRIVED!

- Prior to receiving a flu vaccine you must complete an online survey (consent) in your Readyset Portal
- Employed staff (those who are paid by LCMC Health or one of the hospitals) will have direct access to their Readyset portal after signing into LCMC Health by accessing this LINK >>
- Contract staff will need to enter their username and password. If you do not remember your username or password, choose the FORGOT username/password on the login page.
- If you received your flu vaccine outside of the LCMC Health Employee Health/Team Member Health departments, please upload proof to your portal and complete a survey
- Contract staff DO NOT need to provide flu vaccine proof to LCMC Health but to their employer
- Employees with a contraindication to the flu vaccine MUST complete a survey in their Readyset portal
- ALL EMPLOYEES ARE REQUIRED TO SUBMIT FLU DOCUMENTS BY OCT 31
